the technology firm Outcomes'10, installed in Espaitec, Parc, Científic i Tecnològic de la Jaume I University of Castelló, has been developing since the end of 2020 the project SmartChronic to implement artificial intelligence in the healthcare of chronic patients. The project is led by the Joint Unit in ICTs applied to the reengineering of socio-health processes of the Health Research Institute Hospital La Fe in Valencia, Involving nunsys, the Poliwellbeing Research Institute of the University of Valencia and the Informatics Technological Institute (ITI). The initiative is financed by the Valencian Innovation Agency (AVI).
The main objective of the project is define and deploy integrated care pathways for chronic patients complexes requiring long-term care. Contributing to the efficiency and sustainability of the health system and to the improvement of people's health conditions and quality of life. For this, a multidisciplinary action centered on the patient, which uses predictive analytics and non-invasive monitoring techniques to determine the degree of fragility of each individual; and optimize the medical care plans that each patient requires.
La artificial intelligence will allow healthcare professionals and managers have the ability to optimization in decision making, improving the coordination, collaboration and integration of all the agents involved in the care of complex chronic patients.
The Outcomes'10 team, coordinated by Dr. Luis Lizán, analyzes and determines the most important aspects from the patients' point of view, which must be integrated into a predictive tool; and the follow-up and monitoring of chronic patients. The final tools aim to integrate the essential aspects to evaluate the health status, such as the functional, psychological and social status of the patients, and validate it in clinical practice.
